I, Full name grant permission for the County road equipment and/or its agents or representatives to enter upon the following lands:

For the purpose of Road Construction and Backsloping :

I have been asked to move my fence (If Applicable).

I also agree that the County will not be liable for crop damage where road constructed.

Construction may not be completed during the current season due to exterior factors
(e.g.: Inclement Weather)

Top Soil will be removed before construction and replaced after construction.

Any disturbed grass areas will be re-seeded.
